Provided are Medical Health form PDF documents to download, fill out, and take to your personal physician. Please return completed records to your Scoutmaster. Note, it is very important to have current health information on file to participate in scouting activities.

Personal Health & Medical Records - The Troop is required to maintain current personal health and medical records for all individuals who participate in Troop activities. Individual responsibility is expected for the maintenance of personal health records to assist in overall Troop Health Status management. (i.e., Scouts are required to keep an original and current copy of their individual health form.)

NEW ANNUAL HEALTH AND MEDICAL RECORDS FORM - The Boy Scouts of America announce a new Annual Health and Medical Form, which replaces former Class 1, 2, and 3 forms, which will be phased out in 2009. The use of the new form, No. 34605, will be required effective January 1, 2010, as well as for the 2010 National Scout Jamboree.
