Step #4 - Eagle Scout Resources

Listed below are a collection of forms, documents, and misc. files that are intended to assist Eagle Scout candidates.

  1. Preapproval Application for Eagle Scout Service Project - [PDF]
    Once you have selected a project idea, you must discuss your project goals with the beneficiary. This could be a chuch representative, property chairman, school representaive, or community adviser that represents the organization for whom your project will significantly benefit.

    Upon approval of the project idea by the beneficiary, you then need to provide a written proposal describing the project idea. The preapproval application will contain the questions you'll need to answer in order to present your formal proposal to the Troop Committee. Remember, a well throughout plan accompanied by photographs, diagrams, or illustrations will help present a clear idea of your intended goal. The last page of the application is where you'll have the project beneficiary, Scoutmaster, Troop Committee Chair, and District Advancement Chairman all sign off for approval before you can begin initiating the project. Note: The preapproval application will be returned to you by the District Advancement Chairman, (Mr. Tom Fonda), usually with an explanatory letter that is also sent to your Scoutmaster.

  2. Eagle Scout Leadership Service Project Workbook - [PDF]
    The Eagle Scout Leadership Service Project Workook is an excellent resource tool that can also serve as a project preapproval form. This document assists the scout by allowing the candidate to type his information right into the editable text blocks and documenting all the factual information needed when developing the final report.

  3. Eagle Scout Rank Application Form - [PDF]
    This form is one of the last, but most important documents needed to process your Eagle Scout advancement. This form can be filled in during anytime of the Eagle advancement process and it is strongly recommended that the Eagle Candidate use a working copy of the application to fill in dates for merit badges, service project completion, rank advancements, etc. Note: Be sure that the dates are correct and that each of them includes a month, a day, and a year.

    By benefit of good Troop organization, all registered scouts have an ongoing individual history report that reflects BSA enrollment, rank advancements, service hours, merit badges, and board of review dates. Either Bob or Marge Sutherland from Troop 380 can be contacted to obtain this information.

  4. Eagle Scout Rank Application Form Requirement #6 - [PDF]
    This form is an addendum of the Eagle Scout Application and requests the Eagle candidate to breifly describe his ambitions and life purposes. It also requests the candidate to list leadership positions in regilious organizations, school, athetics, boyscouts, and any other organization the scout may be affiliated with. Note: It is a good idea to provide this form as a separate piece of information and should be included with the final Eagle Scout Report in addition to the final Eagle Scout Application.

  5. Confidential Reference For Eagle Scout Award - [ PDF ]
    Upon filling in the information for your Eagle Scout Rank Application, you'll be asked to list six references of individuals who you know personally and would be willing to provide a recommendation on your behalf. In addition to listing these individuals on your application, you'll need to submit to them a Confidential Reference Form for their review with direction to be mailed back to the Troop 380 Advancement Chair, Mr. Robert Flipse. Note: It is customary to provide a self-addressed envelope with Mr. Flipse's return address.

  6. Application for Alternate Eagle Scout Rank Merit Badges - [ PDF ]
    This application includes the necessary information to properly apply for alternate merit badges on the route to Eagle Scout.

  7. The Eagle Service Project Report
    This Web site is an excellent resource to begin to think about just how to write up your final report. Remember, Mr. Tom Fonda has very clear suggestions for what he is looking for. Simply, a "Word Picture" is great way to think about the initial process. Using your personal log will help you organize you material.
